Kindred Transitional Care and Rehabilitation - Canyonwood is dedicated to meeting the specific needs of every resident and patient we serve. We are changing attitudes about nursing homes by offering care that extends beyond traditional custodial care.

We tailor our programs to individual needs. Upon admission to our center, all residents and patients are assessed to determine what care is required. Treatment programs address areas related to the mobility, performance of activities of daily living and communication skills of the patient. The staff at Canyonwood Nursing and Rehabilitation Center also treats a variety of patient and resident conditions including orthopedic, neurological, degenerative, and communication disorders.

Our Alzheimer's program is designed to celebrate the spirit of our patients and residents. We strive to offer a comfortable and compassionate environment that emphasizes positive life experiences and active involvement in a daily routine. Our staff caring for our Alzheimer’s residents focuses on meeting their special needs. We encourage family to remain integrally involved in the lives of their loved ones and offer ongoing support through individual care plan conferences and support group meetings.
